If you are completely unable to login with a QR code, you may want to confirm that the camera has been enabled for the specific app you are trying to launch.
To check the app permissions (camera, mic, eye tracking, etc.) open the Settings app in your headset. Then select Apps and wait for the list of installed apps to populate. Find the app that's giving you trouble, tap it in the list, then tap on Advanced Options. Confirm that all required permissions are toggled on.
